Immunization Requirements

Marian University Required and Recommended Immunizations and Screenings

Required immunizations and screenings

  1. MMR series
  2. Tetanus-diptheria (Tdap), received within the last 10 years
  3. Meningococcal (MCV4) series
  4. Meningococcal B series
  5. Varicella series
  6. Hepatitis B series
  7. Polio immunization
  8. Tuberculosis screening is required for all international students within four weeks of arrival to the United States and for students with TB symptoms or risks.  Please upload your TB screening results with the attachment tab located on the bottom of the immunization form.

Recommended immunizations

  1. Annual Influenza vaccine, beginning in OCT for the upcoming flu season
  2. COVID 19 series
  3. Hepatitis A series
  4. Gardasil immunization

The Marian University Student Health Center offers the following immunizations and screenings for a fee:

  • Tuberculin PPD (Mantoux) test 
  • Quantiferon Gold TB test
  • Titers: Hepatitis A, Hepatitis B, Hepatitis C, Varicella, MMR
  • Tetanus toxoid, reduced diphtheria toxoid and acellular pertussis vaccine absorbed (Tdap)
  • Annual influenza vaccinations

Health Insurance

Marian University requires each student hold an active health insurance policy.  Patient health insurance must be provided when appointments are scheduled with The Student Health Center, as immunizations and some appointment services can be billed to a patient's health insurance.  TB (including Quantiferon Gold) tests and blood titers are not billable to health insurance and patients are responsible for the fee at the time services are rendered.  If the patient does not have health insurance, or does not provide health insurance upon scheduling the appointment, the patient is responsible for the associated immunization fee at the time of the appointment.

  • Patient first and last name
  • Patient birthday
  • Health insurance company
  • Health insurance claims mailing address
  • Health insurance customer service phone number
  • Member/policy holder number
  • Group number
