San Damiano Scholarship Application and Nomination Form
Application and nomination forms must be postmarked by December 31 , 2007.

Step 1:	Apply for admission to Marian College If you have not yet applied for admission to Marian College, please do so as soon as possible. You may apply online at no charge.
Step 2:	Apply for the San Damiano Scholarship Apply online or download the San Damiano Application Form. Mail the printed application to: "REBUILD MY CHURCH" San Damiano Scholarship Marian College 3200 Cold Spring Road Indianapolis, IN 46222-1997 All application materials must be postmarked by December 31, 2007.
Step 3:	Get two nominations To complete your application, please have two individuals complete the San Damiano Scholarship nomination forms. At least one of your nominations should come from someone who can attest to your participation in community and church leadership activities. Each individual can submit their nomination online at rmc.marian.edu/sandamianonomination.asp or they can download the nomination form and mail it to: "REBUILD MY CHURCH" San Damiano Scholarship Marian College 3200 Cold Spring Road Indianapolis, IN 46222-1997 Nomination forms must be postmarked by December 31, 2007.
